Antwi-Adjei started his youth career at Hasper SV and in 2009 he was spotted by MSV Duisburg scouts. The 171-cm-tall midfielder played for die Zebras before joining Eintracht Dortmund in 2011.
One year later Antwi-Adjei moved to Westfalia Herne and he continued playing for the club at senior level. In 2014 he signed a contract with TSG Sprockhovel and after scoring 24 goals in 89 appearances for the club he moved to SC Paderborn in 2017.
Over the years he has established himself as a first-team regular at the Benteler-Arena outfit, scoring 10 goals and providing 3 assists for his teammates in the 2018/2019 2. Bundesliga season. His main position is left midfield, but he is used a second striker as well. His preferred foot is right and his contract with SC Paderborn expires on June 30, 2021.
